Responsibilities

Provide medical service to assigned patients and coordinate aspects of care within the parameters of specialty including but not limited to:

  • Act as treatment team leader to develop an appropriate an appropriate rehabilitation treatment plan.
  • Provide appropriate patient procedures, achieves patient outcomes, provides for patient comfort during medical and surgical procedures, and maintains the highest standards of patient education, infection control, and safety, patient satisfaction, and quality of care; communicates effectively with patients and their families
  • Evaluate appropriateness for Inpatient Rehab level of care and, if not appropriate, makes alternative recommendation. 
  • Leads the interdisciplinary team to establish a written plan of care at admission and intervals specified in the plan. 
  • Participation in on-call coverage. 
  • Participation in the follow-up care of patients in the SH outpatient clinic as directed by the PM&R Medical Director. 
  • Understands Lean principles and utilizes Lean tools to improve care 
  • Participate in the development and implementation of clinical pathways, protocols, and other clinical effectiveness projects 
  • Participate with the Hospital’s Care Management Coordinators, Discharge Planners and staff in maintaining effective utilization and resource management, medical record appropriateness, and appropriate discharge planning. 
  • Participate in patient rounding, triage and disposition to expedite patient flow throughout the organization and assist with patient placement and census management efforts. 
  • Participate as requested by the PM&R Medical Director, in periodic team meetings, email, and/or other communication methods to foster effective working relationships and reduce variation in practice patterns. 
  • Perform such other services as are reasonably assigned by the PM&R Medical Director. 
  • Participate with team members to assure providers within the rehabilitation unit meet the rehabilitation patient satisfaction metrics annually. 
  • Model, mentor and work with other rehabilitation team members to assure continuous improvement in patient satisfaction.
  • Maintain excellent working relationships and communication with the other physician members of the Physical Medicine Program, support personnel provided to the Rehabilitation Program, Hospital Administration, and Salem Hospital Medical Staff.
