Athletes Grades 5 -12
All student athletes competing in middle or high school sports MUST have a sports physical on file prior to starting practice.
Parent signature required on Pre-Participation Liability Form and Sports Physical Exam Form for anyone under 18.
Parents do NOT have to be present with signed consent.
